There are limited timely care and rehabilitation services for patients who have suffered from strokes and fractures. Many families have to send the discharged patients to private elderly homes. The project provides transitional day care services and high intensity rehabilitation training to help these patients recover better and defer their needs for institutional services. The services are provided by a multi-disciplinary team consisting of physiotherapist, occupational therapist, nurse and social worker. The project will generate income from the services while providing discounted rate for low and middle income families for the service sustainability.
